Agency Producer Agreement including Exhibit A and Exhibit B Effective April 1, 2007, the FWCJUA will move to an Agency Producer Agreement to replace the Producer’s Agreement. Please select this link to complete an application for authorization to submit business to the FWCJUA. Upon completion, include the following with your application:

1. A copy of the Agency’s Certificate of E & O Insurance in an amount not less than $500,000 per occurrence;
2. A check made payable to the FWCJUA for $100 for each Designated Producer identified in the Agreement. (Note: one check may be written to include the fees for multiple Designated Producers);

Authorized Producers not electing to enter into the Agency Producer Agreement will automatically terminate 3/31/07. Authorized Producers may continue to submit business with effective dates prior to 4/1/07, and the Producer will continue to receive producer fees through policy expiration or cancellation. Policyholders will receive a non-renewal notice approximately 50 days prior to policy expiration and be advised that their producer is no longer authorized and will have to re-apply to the FWCJUA through an FWCJUA Authorized Agency.
